Port/Date of Entry on AR-11

What should I put for the Port of Entry and Date of Entry
on the address change form AR-11? My very first entry
date/port into US or the most recent ones?

Would mailing this form by certified mail with a return receipt
good enough for proof of delivery and fast enough to get
to BCIS in time?

The most recent entry into US.

Certified mail is good enough. Also keep copies of AR-11 as filed.

I filed AR 11 a couple of weeks ago.

Under Port of Entry I answered "AOS at CSC"

I hope that's o.k.?
