Police certificates for ds230

I am a US citizen. My wife will be applying for the Immigratoin Visa using the ds230 form. It asks to get
police clearence certificates. My wife has lived in 4 places within India for more than 6 months from when
she was 16 years old. Does that mean she would have to get a police certificate from each of these places?
Or is it enough to get a police certificate from the passport office and the district police of her currently
residing area.

Is there anyone who filled in the ds230 form and had to get police clearance certificate in the forum? When I ask the NVC, I get 3 different responses,

1. Need PCC from district police from all places she has been in for more than 6 months since 16 years of age. (My wife has stayed in 4 different states in India from when she was 16 years for more than 6 mts)
2. Need PCC from Passport office and PCC from the local district police office
3. Only need PCC from the local district police office

If any one has done this, any information would be good. Please help.
