If you are in USA get the police certificate from Indian Consulate/embassy. Less hassle. But takes upto 45 days. Check the relevant section at www.immihelp.com

If because of time constraints, you are trying to get it directly from India, you will need two certificates -

1. from the district police station serving your area of residence
2. from the Regional passport office which issued your passport.

This is what everyone knows and this is what you will find at the website of US embassy in India ....but i downloaded a PDF document (dsl1083.pdf) from some website that i was visiting (don\'t remember which) and here is what it says -

This document is titled IMMIGRANT VISA SUPPLEMENTAL INFORMATION SHEET. Note that as per this document you need a police certificate from all the localities you have ever lived in india since 16.

"Police certificates are required from every visa applicant aged 16 years or over for each locality of the country of
the applicant’s nationality or current residence where the applicant has resided for at least six months since
attaining the age of sixteen.
Police certificates are also required from all other countries where the applicant has
resided for at least one year. Generally, application for such certificates should be made directly to police
authorities in the district in which you resided. If you have any questions about where or how to apply for police
certificates in other countries, you may communicate directly with the U.S. consular office processing your visa
applications.
Do not attempt to obtain police certificates covering residence in any of the following countries, as they are not
available:

in person.
