If I'm not able to go to the US Consulate on the date of appointment, can it be cancelled in advance?
Also, once I get all the relevant docs from my employer, is there a defined timeline within which I have to get my H1 stamped? Can I have a gap of 3-4 months between getting the docs & going to the consulate for stamping due to medical reasons?
Pls help as these issues are very important for me. Thanks in advance.

Is that your first H-1B? Yes you can get it stamped after the start date mentioned on the form I-797. However, you should obviously go for stamping when there is sufficient time (say 6 months) remaining on the approval notice. Please note that once you are on H-1B in the U.S. you need to be paid, otherwise threre will be a violation of status.
