Pls Help : change address form G-731

Hi folks
I am waiting for plastic card for me and wife, have changed address, so filed AR-11, called 1-800#
Also I need to fill G-731. Can anyone please help in filing form G-731, actually my Lawyer is asking $1000 fee, looks like a bit ingenuine to mePlease help

Looks like I need to keep page 1 of G-731 blank, so I don't need to fill anything for Section A and B on page 1 ; right?

Really appreciate if u can tell what to fill for these columns on page 2:


Form Type:
Receipt Number(Is it LIN.........?)
Date Filed: Is it the approval date or notice date on I-485 recepit


Q2) Do I need to file separate form for my spouse also

It would help if you mention about your Service center clearly:)
Anyway, you still have to fill sections A and C of first page.

Form type is I485, receipt number, Notice date.
Separate applications for you and your spouse.

My city and State is Cleveland, OH & I come under Nebrasca Service Center(NSC)
My case is approved and I have Moved from Cleveland OH to NJ(VSC)
My case is approved and waiting for plastic card.
I have filed AR-11 and called 1-800 # for change of address.
Someone told me that I also need to file from G-731 also, if I want my plastic card to be delivered at my new address.
Can you please tell me that do I need to file form for VSC also

You don't have to file AR11 separately at VSC. Also call the Customer Service number and update your address.

G731 is for enquiring about I551 which is renewal. In your case it is not a renewal, a mere address change request (AR11) should do.
