PLEASE !! Two questions for the form I-485 ??

Part 2. I am applying for an adjustment to permanent resident status because :.... and then there's 7 answers + 1 "other". Knowing that I'm filing for AOS through marriage, and filing the I-130 concurrently, what should i check ??
Then, for the Part.3, question 2 :Have u ever received public assistance in the USA (...). I was pregnant in 2001-2002 and applied for KidCare, never paid a visit to the doctor, and didn't pay the hospital etc,... Is it a positive answer ? Should I check "yes" ? And if so, what documents should I provide ?
Please, I need any input on this, because I am about to send my package, and I really want everything to be right..
