From my understanding, you should hold H visa if you want to apply for LC, actually, your employer apply the LC for you. Your employer must prove that there is shortage or unable to recruit american employee for a certain amount of time. Your employer is the key player of the LC application. I think as far as your qualification, you should hold H visa and medical license.
LC is different from the path of NIW. NIW could be self petition based on working in HPSA.

you cannot apply for labor cert based on your employment as a resident since residency is not considered a "permanent position". Previously people used to get GC based on NIW during residency if their hospital was in an underserved area. If you are currently on H-1B ( and not on J-1) and your hospital is in a qualified underserved area you can try to apply for NIW right away. otherwise you need to wait till the point you find a "permanent" job and apply at that time. labor certification can only be done for "permanent" positions.
