Please provide sample letter to Congressman

Hello Everybody,
USCIS generated RFE(wife's) on august 5th. I did not received letter from them till now. I am calling for every 30 days and requesting non-delivery RFE. But till now I did not receive any response from them. Can anyone provide sample letter, how to authorize congressman/woman to make inquiry regarding my case. Can any one provide me a sample letter...so that I can understand procedure. I understand every case is unique. I need just format..

They have forms that you need to fill up and fax. Usually at their websites, at "Contact us" or "Sevices" etc.

There is no specific format, when I wrote a letter to senator's office to find out about the status of my dependent's case.
I just gave them both our LIN#'s, A#s and asked them to follow up with the USCIS and let me know the status

They did respond to it. They forwarded my query to the Congressional Liaison at the USCIS and they responded with the status to the senator's office who passed on the details to us
