Please provide info of CSC Level 1 inquiry for I-485

We mailed I-485 in April ,2001(RD=4/26/01,ND=6/26/01). FP :10/01,11/01(got 2 FP notices, did both). AVM: "12/27 resumed...." I guess we can think of sending level one inquiry now. Anybody knows the name of the form and where to look up to download the form? Thank you very much in advance!

Thank you for your response. It is very helpful. I saw many
approvals with May even June RD s and began to worry about
our case. Well,I guess I will send fax and wait. Thank you again.

Taiked to IIO

Thank you for your kind response. I Talked to an IIO one week
ago. She said My case had been assigned to case officer and it
took 60 to 90 days to get result. I remember a guy posted that a
IIo told him him his was assighed to the officer and took 60to
90days....., he was still waiting after 90 days. So I hope what she
said was true and we will get approved 2 or 3 month later.
