employment letter shold mention following three key thing:
1. date of hire
2. current title
3. current salary
Ask youtr HR. They knows better.

regarding employment history, you just have to specify the date of hire, termination date. for example,

04/15/2002 - 10/31/2003: XYZ Inc
11/07-2003 - 06/20/2004: Doe Inc.
