Please help - very very Urgent!!!

Hello everyone:

I got an RFE and it is asking "your application contained a medical examination of aliens seeking adjustment of status (form I-693) and the supplemental form to form I-693 (immunication record). The record indicates the applicant had a medical examination as evidenced by form I-693, but the civil surgeon completing the forms failed to complete the supplementan form (immunization record).

Now the problem is the doctor is no longer practicing in the same hospital I went to get this done in 2007 and also I relocated to different state. I can call his office and ask them to send me the record. Is that going to be ok? Or should I go find a new doctor (different state) and show my immunization record and get a report from him and send to the USCIS? Please help me ASAP.

Kenosha, the ref that you received will also have a sealed envelope addressed to the DR. who did the medical. Inside that sealed envelope will be the original immunization form submitted by the doctor. Also the ref letter you received will state clearly that you are to take the unopened envelope along with the Ref letter to the physician who did your immigration medical. Do not open the envelope. It should not be too difficult for you to find the DR office where ever he is. It may be costly but its worth the effort. Give him the sealed envelope and the Ref letter. Make sure you keep a copy of the ref letter for your record. Remember there is a time limit with which you have to supply the requested information. Dont delay and give immigration a reason to close your case you have been waiting too long to let a simple matter like this delay your approval.

I received a letter saying they need the immunization record filled by doctor and send to them and also it was talking about "gold sheet" with the record and I do not have that gold sheet. Any idea what they are talking about?
 
Got the medical immunization record done with a different doctor today (replied to RFE). Is this a good sign? So I can hope to get the GC soon? Expert? Any comments?
