Please help to fill EAD form...

Hi Friends,

I would like to apply for my wife's EAD renewal. I have following questions about the form.

1.
14. Manner of Last Entry (Visitor, Student, etc.)

What should I write here? She went to India 2 months back and back on Advanced Parole.

2. Do I need to fill G-28 form?

3. Signature of Person Preparing Form, If Other Than Above:

What should I write here? I am Preparing this Form for my wife.

4. Do I need to send a signature card? When I renew my EAD last Dec, I did send it. But now, If you go to INS website, it says that,

Please note: The blue "Signature Card" mentioned in the form's instructions is no longer required by INS.

Also I would like to know the fees for AP and EAD renewal. Do I need to take casher's check. Is personal check is OK???

Write down just "Advanced Parole (I-512)"

Nope, you need to fill only I-765

Normally this part is for attorney. As you are filing yourself, assume that your wife herself is filing. Leave that part blank. Just fill up the form and get it signed by your wife. That's it.

4. Do I need to send a signature card? When I renew my EAD last Dec, I did send it. But now, If you go to INS website, it says that,

As it mentioned, signature card is not required (new EAD cards do not contain signatures anymore).

In addition, send photocopies of her old EAD card (if any) - both sides.
If you retained the form (I don't remember the number) which came with her earlier EAD card, send the upper portion of it with application. It's basically the paper on which the EAD card was attached.

Send photocopies of the DOB and marriage certificate too.

Congress reinstated the old fees. It may be effective anytime. Probably it's a good idea to send the old fee - $110 for AP and $120 for EAD.

Yes, INS accepts Personal Cheque also.

I don't think so. In the contrary, it will be waste of time if your applications comes back due to underpaid free (just for few bucks). CSC accepts personal checks (through they are very slow in processing but very prompt to encash fee.:) )
