Please help - RFE for birth certificates

Hi

I got an RFE for my parents birth certificate. I am getting affidavits from their siblings for the same since there is no record for their birth dates. Is the following format fine ?

AFFIDAVIT BY ABC
RE: DATE OF BIRTH OF xxxx

I,ABC, hereby depose and say.

I, ABC , was born on ........................

My full and complete address is - ABC street...

I am closely related to xxxxx as I am his elder brother.

xxxx was born on date... at place... to mother yyyy and father zzzzz

I have direct personal knowledge of the event and circumstances concerning the birth of xxxxx because as his elder brother we grewup together in the same house with our parents yyyy and zzzz ---- ANY OTHER DETAILS HERE ??

I have known xxxxx since his birth on date....

This affidavit is submitted because there is no official record of birth of xxxx
I declare under penalty of perjury that the forgoing is a true and correct statement
 
Format is good but make sure that all names are spelled correctly and consistently in all your filings. Two affidavits are required for each person and they need to be notarized.

You will also need an original letter of non availability of birth issued by the registrar of births (or equivalent government authority) if they don't have birth certificates.

For the non-availability, since they were born in villages before the partition, I can only get the sarpanch's sign (government equivalent for a mayor). I hope that will be enough

Yes, a letter signed by a village sarpanch should be sufficient. It should be on official stationery with seal and should state that a search was made and birth records for person XXX, son/daughter of mother YYY and father ZZZ are not found in their records.
