If ur referring to Item no. 1 in the check list (Appendix \'C\') then it is 3 copies of form IMM0008. Fill one form and take 2 photocopies to make it three.

Three forms?
Are you sure it is 3 copies of Form IMM 0008 and NOT 1 copy of (Form IMM 0008, Form IMM 5406, Form IMM 1052) per person?

I actually received a hard copy of the forms from Buffalo. In my packet, they had changed the 3 copies to 1 copy, so I sent in only 1. I received my AOR and I think AORs aren\'t issued for incomplete apps. In any case, my forms said 1 so I sent in 1, if your says 3, just send in 3 of everything...better to be safe than sorry!

require education and field of experience be similiar
I applied PR last fall, I had BS, MS in Electrical Engineering although I took some CS course. My working experience is in both field. Two years in EE and last three years in Software Engineer. Do you think this will cause the problem. Since in old regulation require education and field of experience be similiar. According to my lawyer, my case should be assessed under old regulation.
Please shared your opionin.

Forms Requirement and other questions
Hi ALL,
Can you please help me with what forms should i send for
Canada PR while applying for independent immigration from US?
I am applying the forms i downloaded from internet. Should i
have to send 1 copy of forms IMM 0008 and IMM 5406 for myself
and another same set filled by my spouse, and is this sufficient?
I couldn\'t find the other form IMM 1052 (a typo?) as noted above
in a response. Can anyone tell me the correct one if there is any.
Another question. The Buffalo instruction checklist asks for
previous employer experince certificate detailing the salary,
period of employment, job description, positions/time spent.
I am no longer able to contact my employer in India as the
company nolonger exists (4yrs back). I can get the required
letters from later ex-employers. So my question is it sufficient
if i show just a salary letter that employer? Will there be any
chance of rejection? Should i enclose a letter that i cannot
find the employer and hence i cannot enclose all the details.
Any inputs are appreciated.
