Please help on filing EAD

Hi,



Can someone tell me what docs are required for filing an EAD? The i765 form does not indicate clearly what I should provide to them.
Also the form I got seems not right because it does not have a last name entry and also does not allow to to enter the birth date. Can someone also forward me to link to pick it up?

form I-765 (rev 5/09/02) i think earlier rev also works download it from ins web site

2 photo (same as ur 485 app)
copy of 485 receipt
copy of ur old ead card both side (if it is renewal)


Family name is your last name in the form(itm 1)
item 6 is date of birth

I appreciate your help. What I meant was the item 6 does not allow me to enter from the keyboard. But I will use pen.
