Please help me -- J1 to H1b

I entered into the US as a J-1 visa holder on Aug.1, 2004. Now I'm preparing the documents for my H1b visa application. I have several questions to as you, and I would be much appreciated for your answers.

1. What are the alien number and current I-94 number?

2. What is the I-94 current expiration date? Please note: I entered into the US on Aug.1, 2004

3. Are applications for replacement/initial I-94 card being filed with this application? YES or NO? I have never applied replacement/initial I-94 card? An I-94 was given to me when I was on a plane. I'm confused by this question.

4. Are applications by depedents being filed with this position? YES or NO? I can not understand this question.

5. Is any person in this petition in exclusion or deportation proceedings? YES or NO? I can not understand this question.

1. An alien number starts with an "A". You probably do not have one, so enter "N/A" or "none". The I-94 number is the "Departure Number" in the top-left corner of your I-94 Departure Record (the part of the I-94 you got back from the immigration officer at your point of entry).

2. The expiration date is witten on your I-94. It may be "D/S". Should be near the stamp where it says Class/Until (class with be J1).

3. "NO", unless you have lost your I-94.

4. Do you have wife/kids who will need dependent visas? If not, answer "NO".

5. I am pretty sure you would know if you were in exclusion or deportation proceedings, so answer "NO".
