My mom applied for expension over a year ago and received notice that it has been approved. However, it had no date on I-94. There was only a date when it was issued and no date of validity. The filed was just blank. What does this mean?
We have sent numerous letters asking them for explanation but did not hear from INS.
I have been told that it may mean that it was given up to the date of issue but it sounds ridiculous since we received it after that. But the lawyer I spoke to was confused too and did nto know for sure.
Anyway, several months passed and we don't know what to do.
Can we submit a new petition for extension?
She doesn't want to fall out of status since she wants to come back.

I would take it as an extension for the total time of 6 months. A b2 usually gets 6 months, but can be given a day short of a full year (364 days).
