Please help! I-140/I-485 forms

Hello all,

I would highly appreciate you can help my clarify following questions. I am going to send out my package soon...

1. Should I pay fingerprint fee together with I-485 fee? Shall I put the total payment of 385 (70+315, right?) on one check or two seperate checks?
2. Can someone help confirm the selection of the "Application Type" question on I-485, is it "a" or "h"?
if it is "h" (other eligibility), what I shall put on the explaination line?
3. My spouse is currently on H1 and I am filing I-485 for her. Do I need to provide I-134 for her? Some people say it is not required if the spouse is working, as long as she can provide the recent paystubs.
4. My spourse had a Alien # when she applied the EAD card for OPT, does she need to fill the Alien # on the I-485 form using this number. But the thing is her OPT has expired already, so her A# is expired too, right?

1. Yes. Make one check for each person's I-485 and write "I-485+Biometrics" on your check.
2. "A" for you, "B" for your dependants.
3. It's not required in the instructions for I-485.
4. You can write the number and its origin (expired EAD). Probably nothing bad will happen if you do not write it.
