Please Help ... H1B EXTENSION ...I-797s FROM PREVIOUS L1 VISA

EXPERTS, PLEASE ADVICE. THIS CASE IS RELATED TO H1 EXTENSION.

First time I came to US in 2003 on L1 visa for a period of 1 year. My L1 visa is approved on BLANKET L1-VISA category and hence, I was given a geneirc I797 form which won't have my name on it.

In June 2004, I have applied for H1B conversion and I have got my H1B approval(I797) in September2004. But, without shifting to H1B, I applied for the L1 extension in September 2004, as my L1 I94 is expiring in Dec2004 and I continued working on L1.

In November 2004, I was approved my L1 extension and I was given L1 extension including I94 valid till Dec 2006. This time, I have got the I797 of my L1 visa on my name.

I went to India in January 2005.

I have got the H1B stamping in April 2005 in Delhi on my H1B visa.

I came to US on H1B in May 2005.

I have got my PERM process approved in March 2006 on my H1B visa.

My H1B is going to expire in April 2007.

Question#1:
When I apply for H1B extension, Is it compulsory that I should submit my I797 forms of my previous L1 visa in addition to my I-797 of my current H1B Visa?

Question#2:
Will my H1B Extension be denied if I do not submit the I797 forms of my previous L1 visa when I apply for H1B extension?
