Please help: emplyment letter format

I'll be interviewed soon, and i'd like to know the most standard formt for the emplyment letter.

(Question #1)
Please write sentences for an applicant (xyz) who is emplyed in company (ABC).

(Question #2)
By the way, who should be the author of the letter ?? My boss ?? or Human Resource department ?? This isn't clear to me as I'm employed in academy (not in industry), and I used the letter from my boss (Lab head) when I submitted the original application, but it seems to me that most people here get the letter from HR.

You can ask you Lab Head to give it on the Academy letter Head.
Make sure you list out the duites which should be similar to your Labour

lab head or secy is okay

in academia, funding is not through HR but the department funds (grants etc). so the responsible person would be the PI or dept chairman.
I did the same.
