Please help: EAD renewal

Hello,
I am planning to apply for renewal of my EAD based on I-485 pending adjustment status.
I go the following list somewhere in this forum as required documents to send to CIS:

• Check in the amount of $175 for the filing fee;
• Copy of front and back of I-94 arrival/departure card and latest H1B approval notice;
• Copy of front/back of last EAD and previous EAD mailer;
• Form I-765 with two photographs;
• Copy of I-485 filing receipt notice;
• Copy of I-140 approval notice;
• Color copy of passport's biographic page;
• Copy of Birth certificate.

Now I was reading I-765 (OMB No. 1115-0163; Expires 04/30/05). I have following questions:
1. Is this the latest I-765 form?
2. It mentions $120 as fee, am I reading it wrong?
3. It does not mention Color copy of passport's biographic page or Copy of Birth certificate, do I really need to send them?

Any help will really be appreciated. Thanks for help in advance.

1 & 2. Yeah - the form is the latest - and the fee information is incorrect in the form. The correct fee is $175.

3. Not necessary to send color copies ... Its more important to send clear copies (b&w would do) ... you can also send slightly enlarged copies (about 125% or so) ... all they care to see is the photograph in those copies.
I've never sent B.C for EAD .... but I've sent marriage cert for the dependent's EAD. Shouldn't hurt sending BC copy if you have it.

Also previous EAD mailer is good to send if you have it .... but have not seen ill-effects of not sending it. If you don't have it, don't worry too much.
 
uscis says -

New Fee: If your application is filed or postmarked on or after April 30, 2004, the required fee is $175.
