plastic card question

Gurus,

After the 485 approval, what triggers the plastic card issuing process? Does 485 approval automatically add the person to the queue? I also heard that after passport is stamped, it is the local office that requests a card. Which of those two is true?

The way I understand it, when you go in to get your passport stamped, they will fill out a form that you sign. That same form is also used to collect one of your fingerprints. That form is then sent to NSC and this triggers the card orderig process.
