plastic card photos question

My wife & I got our passports stamped on Sep 25. The officer complained about my photos (there were a couple of white spots on it - polaroids taken at motofoto) and said INS may require new photos.

Now, online status says card has been ordered for my wife (Oct 25), but in my case, still shows the 'approved' message.

Any idea what the process is, for sending new pictures? Does INS generate an RFE and I respond to it? Anyone who has had a similar experience?

Also, can someone confirm that permanent residents need not fill I-94 when they return to the US?

I cannot answer your first question, but in answer to your second one, as a US permanent resident, you DO NOT have to complete an I-94 on your return to the USA.
