Pl. help me to file EAD and AP form

Appreciate an advise from Gurus.

I am in the process of renewing my EAD and AP by my self.

1. Wife used her EAD, visited INDIA and came back using AP.
What should be her "Manner of Last Entry" in EAD form??
H4? AOS? or 485 pending????

2. what kind of documents needed to attach with AP form?

3. what is the mailing address? (Filed 485 from vermont cnt.)

You can file EAD and AP extensions yourself

Just make sure that you enclose the following along with covering letter to :

Immigration and Naturalization Service
Vermont Service Center
75 Lower Welden Street
St. Albans, VT 05479.

For EAD extension:

1. Form I-765 and signature cards.
2. Form I-797 D.
3. Copy of previous EAD card.
4. Copy of I-485 fee receipt notice.
5. Filing fees.
6. Color photographs.

For AP extension :

1. Forms I-131 with supporting letters.
2. Copy of I-485 fee receipt notice.
3. Copy of previously issued advance parole document.
4. Filing fees.
5. Color Photographs.

Manner of last entry is 'Advance Parole'

Please type 'Advance Parole' under manner of last entry. This is what my attorney's typed for me.
 
Mail both EAD and AP to Vermont

Forgot to mention in my last post. Both EAD and AP applications should be mailed to the same address in Vermont.
