PIO card

Anyone applied Person of Indian origin card. I am planning to apply one for my baby. Am confused how many photos are needed.
The embassy of india application form says attach 4 photos.
Also on page 1 it says paste a photo.
Also says attach form as duplicate

so I am not sure if photos needed are
a) 4
b) 4 + 1
c) 4 + 1 + 1
d) 4 + 1 + 1 + 4

The correct answer is
a) 4

Paste one photograph to the original application form(on page 1 - wherever it says "Paste a photo here", similarly paste one photograph to the duplicate and attach two photographs(with a paper clip). That makes it a total of 4.
