PIO Card Questions - Houston

Hi All,
I and my husband are applying for our kids PIO Card. We both are indian citizens and applying for PIO card our kid born in US. We are applying at houston CGI.
Could some one please help with these question.
- How many pages of my and my husbands passport do we need to get notarized. Just the first 3 and last 3 will do, or all printed pages are needed.
- How many pages of my kid's US passport are to be notarized?
-On the PIO form Q13,
13) Whether the applicant or either of his/her parents or any of his/her grand parents or great grand parents was born
and was permanently resident in India as defined in the Government of India Act, 1935 * at any time. If so, following
details may be indicated:
Date of birth: __________________________Place of birth_______________________________________________
Proof of residence etc: _____________________________ (Enclose photocopy of relevant documents; Note: If
applicant is the spouse of a citizen of India or a person of Indian origin, his/her particulars should be indicated).

Can someone please tell us whose details are needed?
