PIO Card question

Guys:

Need help please. Applying for a PIO for my newly born kid. Got the kids passport and now applying for the PIO card. Thru this website realized that it needs a copy of my kids birth certificate. Do they need the original or copy of the birth certificate. I tried to take a copy of the birth certificate and its coming all black. After some investigation, realized that a copy cannot be taken of the birth certificate.

Please help, as it needs a copy. If yes, then I may have to shell extra 12 bucks to get a copy from the Health Department.

I tried to scan, but no use. I guess these people use special paper and color which doesn't allow to print or make a copy. I went to the HD office and purchased a copy their directly.
 
Which state or local authority are we talking about here... at least in Massachusetts, one can get it from the State Vital Stats, the city the child was born and the town where the parents reside. The latter two town could be the same. All use different paper and formats, but could make a certified copy the original record on their paper as well.
 
I am talking about Florida. I enquired and it seems that we simply cannot take a copy. I have to purchase a copy from vital stats office in FL. Also I have another question. on the BLS web site, they are asking us to make two copies for submitting for PIO card. Do we need to attach two copies of all document?

Please let me know if anyone has recently applied.
