PIO application preparation

I am preparing to apply PIO for my wife. See my other question:

My Wife was born in India and remained Indian citizen. She recently acquired US Citizenship.

In the PIO application, question 10) Does the applicant hold dual nationality.
I think the answer should be NO since she never had dual nationality. She was a Indian Citizen but had her Green card for the past 7 years till she acquired US Citizenship this month. Please confirm.

For Question 13, proof of residence, i think her copy of her old indian passport should suffice. Please confirm.

Also, should we send her original indian passport with the PIO application? Since they did not ask for it, i would like to keep it with me since we are planning to apply for OCI in the future. The only reason we are considering PIO is that it is quicker to get since she has a trip to India coming soon.

Do we have a sample of completed PIO application any where?

Since your wife previously held Indian citizenship and passport, I think Q 13 and 14 are 'not applicable' in her case. Answers to Q 12 should be sufficient to prove her eligibility. I have not applied for PIO myself, but this suggestion is based on my reading of the form at http://www.indianembassy.org/consular/pio.pdf.

And if they are not asking for original Indian passport, why would you consider sending it?
