PERM before filing for H1B?

Hi,
I wonder if anyone can advice me.
I am a physician and have a job offer from employer (University). I am on J-1 visa which is valid until July 1st, and have a favorable recommendation from DOS on my J-1 waiver based on exceptional hardship. Is it possible for my employer to file PERM before I get H1b visa?

That's a sticky situation, since the point of a J-1 visa is a temporary visa where you earn training/etc. in the US in order to take it back to your own country from what I understand.
