passport renew

Hi
Now-a-days, you can renew or get a new passport within one year of your day of expire of that passport. When we send our old passport to embassy office, should you send the I-94 form as well. I wondered for issuing a new passport, whether they need the I-94 forms.
Any comments or experience welcome

I don't think they require I-94.

They didnt take I-94 when

I renewed my pp. They just issued a new one in the evening. you will need to carry both the old and new one since the old one has the valid visa. This was 3 yrs back. check at the embassy website or call them to confirm.
