A activex111 Registered Users (C) May 22, 2007 #2 Hi, I recently got it done.. here's the format that the public notary in india provided to my parents I [father's full name] aged [yr] yrs (DOB xxxxx) resident of xxxxxxxxx, do hereby take an oath and state on solemn affirmation as under That my wife,[mother's name] gave birth to a [male/female] baby on [date] at [time] at [hospital address] I [father's full name] eherby declare that the contents of paragraph 1 and 2 above are true to my personal knowledge and belief Verified at this date [date] at [place] SWORN solemnly affirmed by [signatures and stamp] hope this helps..
