If i don't recieve packe 4 in time before i leave the US and have a family member pick up a copy. Will the folks doing the medicals in bombay accept copies of the appt letter and copies of the medical forms

Yes. The medical folks will accept copies of the appointment letter. The appointment letter should have your name and your case number.

I am not sure what medical form copies you are referring to. The doc/hospital have their US consulate prescribed forms to fill. You may want to take any immunization or any other pertinent doc related tp your health.

Hi Cpers,

Finally got IV appointment . I have not recieve Packet 4 though. Since my wife will leave to our country next week I was thinking to cover myself and authorizing her for picking up Packet 4 at the US Consulate. I was thinking if someone has a template for this letter and if this one needs to be notarized as well.
