P3 Form questions.

I have a few questions that already might be addressed in this forum. But it is a odyssey here, I cant really find them.

Please help me on the following questions based on your experiences.

1. How important is to include my name in my wife's passport or vice versa? Wont marriage certificate be enough?

2. How do I specify my India address for the p4 to arrive. There is no question in this regard in P3?

3. I have a baby that is US citizen, should I include the baby in the list of travelers that I am going to travel with?

{While this has been quoted as a requirement by some consulates, we have also seen some posts which say that they
did not face any problem even though they did not have the
spouses' names annotated in the passports.

If you decide to do this, it is a simple enough service at the
local Indian Consulate. Research the forum for more information}

2. How do I specify my India address for the p4 to arrive. There is no question in this regard in P3?

{You need to communicate this address seperately through
email/phone to the consualate after your case reaches them}

3. I have a baby that is US citizen, should I include the baby in the list of travelers that I am going to travel with?

{Not necessary. Just carry a copy of the baby's passport and
birth certificate to the consulate}
