original I-693 form for medical exam

the doctor asked me to bring an ORIGINAL I-693 for medical exam. I called my lawyer. He said the doctor should have the form or he is not approved by INS. what can I find the ORIGINAL form? you know, I live a rural place. the nearest local INS is over 350 miles:( can I ask a friend to get one for me? Thanks.
 
Check with INS

First of all call INS office ( call this number 800 375 5283, select option 2 for list of doctors and local office numbers). Confirm that you doctor is INS approved. You can download the forms from ins.gov website ( it is in hot topics , \'forms online\'). The doctor shold have these forms, some of them are cazy they don\'t want to spend time for this. same thing happened to me...

My doctor asked me to bring my own forms. When we went there, we realised that he has the forms and already filled out part of them. He wanted us to bring them because he won\'t be running out of forms. (I guess too lazy to get from the INS.) In fact, I happened to see that he has lots of forms in the drawers.
 
You cannot download from the web since this form has carbon copies attached.

Call the toll free no. and get the forms from INS.. It should arrive in a week\'s time. Usually INS doctors have it. Most of them charge a fee for it. But its not uncommon that the INS doctors don\'t have it.
 
Those are old forms

New forms can be downloaded and there is no carbon copies for that.It is valid and INS accepts only new forms.
 
My Dr. Screwed my case

I did my medical examination with a Doctor in my area around Oct 00, I couldn;t find any doctor in my area from the list that my attorney sent me, so I called all the Doctors in my area and checked if they are ins approved doctors, this Dr.with whom I did my medicals said he is ins approved and I took the medical examination with him.
VSC sent me a RFE last month asking me for immunization records, both the Dr. and myself were not aware of this so we didn;t do any immunization originally, I got suspicious with this Dr and called the INS helpline today and found out he is not on the list. But when I called the Dr.s office this morning they are saying they are ins approved and the Drs name is not yet come on the list. I don\';t know what to do, I have already waited for more than 3 and 1/2 years for my case, does anybody has undergone a simillar situate before.

thx. finally called local DOJ (INS is part of DOJ) and the lady there told me INS officer is visiting a town not far from my place now and will stay for 4 days. took a 1-hour drive and finally got it. actually I also went to the INS website and submitted a request for forms there. don\'t when I will get it.
