only green card for travelling

another thread discussed about stamp being valid even when gc comes. my question is can you travel ONLY with green card & no stamp on passport?

some information needed

I got my green card last month and I am travelling to my home country after 2 years.

1. Do I have to carry any documents (like old H1 approvals etc..) other than physical green card & passport?

2. what to do just in case green card or passport or both lost during tarvel outside the US? Just to be aware of the procedure.
