One Question on AP

My wife is going to india she has a valid AP and valid H4 approval notice paper but her visa expired (stamp on passport). My suggestion to her was to go to madras (chennai) counslate and get H4 visa stamped and enter into US using the Visa.
Is it neccessary to get visa or can she just use AP and get back. 1. If AP used will there be any type of RFE generated.
2. Will she have any problem at Port of Entry when she is using AP.

RFE is not tied with using AP, and also she'll not have any problems at POE using AP. The only issue is she'll loose her H4 status, if she uses AP for reentry.
