I have not visited any other country except USA couple of times before. What should be my answer to this question.

I am thinking of putting the following answer.

USA (2003, 2002, 2001)

Would this be a correct answer ? Please advice ...

that's right, but visiting USA 3 times means that you left USA 2 times. also list the countries where you visited after leaving USA. Could be your home country
