One more medical exam question

I am going to file I-485, my lawyer has not give me the INS approved doctor list(company lawyer is very slow), can I just download the I-693 form and go to a INS approved doctor to do the exam by myself? I know a INS doctor from my friend, or I must choose the doctor from my lawyer\'s list? Thanks

You can go to any INS approved doctor to have medical exam. But the form must be original, which have 4 copies with different color. You can get the form by walk-in INS or have INS mail to you.

The INS approved doctors will have the I-693 form. They might charge you an extra $5.00 for that. Else you can get it from the local INS or call the INS toll free to mail one to you.
