On DS-3035 form, ques 13

I have completed PhD from a US univ and currently I am on J1 visa since last 10 months. While filing for J1 waiver in J-1 VISA WAIVER RECOMMENDATION APPLICATION (DS-3035), question no.13 on page 2 asks "Is there any period of time in the U.S. that is not covered by your form DS-2019/formerly IAP-66?"

Does this mean that I need to mention about the time I was on F1 status (prior to the current J1) or is it only to determine if I did not go out of status while on J1. Please advise. Thanks

This is to do with your F1 visa. You may give the information about your status while doing your PhD.

Another quick question. What did you write for # 17. I do not have visa control number for J1 as I did not leave the country ever since I changed the status. So, I will have only date of status change, but not visa control number and port of entry. I have that only for F1. I changed my status from F1 to J1 while being in US and did not leave USA sisnce then. Please let me know.
