If name on us and canceled Indian passport doesn't match exactly NYC asks for document from competent authority or court order. I have read on past forums (there may be a separate thread in same forum about this). Middle name drops are also causing issues.
Generally if there is a name change requested during naturalization, USCIS requests a name change from court and court provides the person getting naturalized a name change court order. I had done this for my wife.
Looks like you either forgot to add middle name to the naturalization process, hence ur US pass and indian pass are different and no name change doc.
I have read a few people advertize in US and indian papers about name change (with ur middle name dropped) and submit this to consulates.... other idea could be change you current US name by going to court to include middle name again, then re-apply for US passport with the middle name then your US pass name and Indian pass name will match.
Please note these are just suggestions from a lay man (not sure if this is sound advice or legal) and there maybe other things people have done to resolve this, so please do check forums and dont rely on my post without checking with lawyers etc.....
