OCI for only Kids

HI,
Myself and my wife got OCI, twoof my kids have PIO and a new born does not have PIO.
I am trying to apply OCI I for all three kids now.
I have filled out the application for only three kids all in one application.
Here are my questions

1. DO I need to go embassy in person?.
2. How many BLS form should I do?.
3. Since tow of my kids have PIO how do I calculate the fee?.

2. One per kid
3. The order form for BLS will determine the fee based on status. My kids were like $145 + fees since they already had PIO
