Birth affidavit;Immunization;naturalization (she does not have indian passport)

Questions;
1.) For my daughter should i be taking Left or Right thumb print ..?
2.) Since B is common for all,
question 19; will have date and method of nationality for both ..Correct..?
question 22; will have only my fathers info is that fine; or does she need somebody in india, if she does how do you associate it with her ..???? because in relationship column it does not say to which applicant ..?
