OCI - CGI-NY Photo requirements help

I am confused regarding number of photos required for OCI application at CGI NY. Do I need just 2 photos affixed on each copy of application or four photos (2 on each application and two extra). I would appreciate if anyone having experience with CGI-NY share this information with me.

Also, do I need to submit PART B for each applicant separately or its one common form for all applicants.

I applied for OCI with one photograph affixed on Part A and sent 3 additional photographs with the application.

My OCI was granted by CGI,NY without any issues.

I applied for individual OCI and not family OCI so can't answer the remaining part of your question.
