OCI and passport cancellation

I am getting ready to apply for OCI and have a nagging question that I can't seem to get an answer on:

Can I send in the OCI appl. along with the photocopy of my uncancelled Indian passport? If so, will this cause some confusion - (because sending originals is NOT required at the time of application)? See item #2 of mangal969's post here. Furthermore, will they know that the original passport has been included specifically for cancellation? Or will they wonder why it's included since there is no specific form for cancelling a passport.

Here are the steps I followed. So far it is working.

1) Complete OATH and become a US Citizen.
2) Apply for US Passport and get it.
3) In the meantime, take backup copies of Indian passport and send it for cancellation. In my case it took 4 weeks total.
4) You don't need to wait till Indian Passport comes back to apply for OCI. Complete the OCI application and send it to Houston CGI.
5) Wait till the processing is complete. I will send my Current US Passport and cancelled Indian passport for the U Sticker and OCI booklet.

Here is my timeline.
1) Oath. 27 Mar 2007
2) Applied 31Mar 2007 and Received 14Apr 2007
3) Sent 31 Mar 2007 and Received 27 Apr 2007
4) Sent the OCI paperwork on 17th Apr. It Reached on 20th Apr. Still witing for Acknowledgement.

How do you send your passport for cancellation? Is there a form for that? How do they know you are specifically sending it in for cancellation? I have looked all over and can't find any info on cancelling passports
 
There is no form. I wrote something like this

Dear Sir/Madam, I acquired US citizenship on <DATE>, and in accordance with the conditions of Indian passports, I wish to have my Indian Passport (enclosed) officially cancelled. Please return the cancelled passport to <address>.

Optional: Enclosed is a return postage paid envelope/return postage of $15
