NSC Fax Number Required

I am not sure why they ask me the proof of my finger print notice along with the stamp from officer to make sure i have done it. Told me its not showing up in their system.

I tried searching where i wrote on small piece of paper, couldn't find it right now from yesterdays crap at my desk.

Any one please...
