Notice of Action regarding medical examination

My wife and I have pending I-485 applications from May 2002.

This week we both received notices that read

Your original Form I-693 is being returned to you at this time. You are required to return to the physician who completed your medical examination to obtain a completed Supplemental Form to I-693 to establish that you are current for all recommended age-specific immunizations or that you have received a waiver of that requirement in accordance with Section 212(g) of the Act. After the supplemental form and the I-693 have been completed and signed by your physician, you must return both forms to this Service. Please note that both forms must be returned to this Service in an unopened envelope that has been sealed by your examining physician.

What is the course of action we need to take? Can I go to the doctor, re-do the medical examination and submit a new I-693 and the supplemental form? Is this what they require from the above notice?

Though it says the original I-693 is being returned, we haven't received any. How long should we wait or do we even need that?

Has anybody seen something like this before? Any clarifications are appreciated.

I don't remember exactly but I think the medical evaluation form has questions about immunizations and whether you have them or have been waived. It appears that your physician did not complete that section. I would say just redo the medical and ensure that everything is complete. The doctor's office usually gives you a copy. Then submit the sealed envelope to USCIS. Best of Luck.

PVM.. I am not sure if you read my posts earlier. 1 month ago i got the same RFE for myself and my wife. Basically, the second page gives you the detials.

What you have mentioned is same for everybody on the first page of RFE. My second page stated", your photocopies of I-693 are being returned please submit the originals".

When i asked the doctors he said they had sent originals, however since i had moved out of that state and i did not want to take any risk i re-did my medicals paid $250/each for my wife and myself. did chest x-rays again as well and returned the RFE.

Just do the required don't get into the hassles of getting originals. If you docotor can re-do everything of no cost to you then it's good othewise go to the new doctor.

I plan to do a full medical again and send both forms (693 and supplemental).

I've given 2nd FP on Dec 03 and waiting to get 3rd EAD renewed (applied in March 04).
