Not yet received visa thru premium processing (3 months)

My employer has filed a H1 visa transfer 3 months ago, but I have not yet received the visa ? Are there any cases such as this ?
They did receive an RFE after about 2 weeks, which was promptly responded to.
Also, they are saying that for premium processing cases, there is no receipt issued, is this true?

the same receipt # continues for the Premium procesisn ie. the receipt #u have already got when you filed your H1-B. hope this helps.

H1 premium procesisng applied at same time

Actually the premium processing was applied for at the same time as the H1- petition was sent.
I have not yet received any kind of receipt for the H1 nor for premium processing
