Non availability certificate , is there any approved form or can it be on a

Guys, please help me out here. My wife does not have a DOB certificate.
I obtained affidavits and when obtaining the Non availability certificate
in a town panchayat in tamil nadu, the officials told us that they dont have FORM 10 and gave the non availability certificate in a white paper with the seal and signature of the sub registrar.

Is this an acceptable format. or should the non availability certificate SHOULD be in FORM 10. please reply my CP is on 27th DEC and I am now in Chennai.

I got the non-availability certificate in form 11 from a registrar in Tamil Nadu.

I have attached the certificate with details blanked.
