New medical form without picture on it

Hi Guys,

I just received the correct medical forms since my last medical forms had a worng name written on it. But the new forms have no picture on it.

So my question is :

Can I paste the picture on the new form by taking it off from the old form or just leave them as it is and submit both the form along with my medical exam?

Please clarify..........

Hi Jalma,

Just leave them as it is.

And you would also require 2 photos for the medicals, you
have to provide them for medicals.

Are the two photos required for medical exam has to be of specific as per canadian immigration or we can use any passport photographs (US size) as well.

Please let me know...
 
Hi cat&mouse...I posted reply on another thread you created.
Here it is:

Just goto any of the listed doctors. They have formB to E. you will only get first page in mail. rest of them ...doctor will fill out. you will need 3 passport size photos, when you go for the medical exam.
