new h1-rfe, what do i answer

1. Names of all employers, dates of employment, and places of employment for the past six years.

2. If employed in the U.S., x's immigration status while working at each employer.

3. If the original date of x's H-1B classification was more than six years ago, provide information requested in 1 and 2 above from that original approval date (I assume that this does not apply).

4. If x's H-1B employment has been intermittent, seasonal, or an aggregate of six months or less per year, submit evidence of that (I assume that this does not apply).

5. Submit evidence of all periods of x's H-1B classification (this would be copies of Notices of Approval/I-94 forms).

guys: what do i respond. i have used up 3.6 yrs of 6 yrs, that leaves me with 2.6 yrs in h1.
i have been employed all along.
my previous company shutdown on may31st and got this job immedietely, but h1 receipt notice is of july22. the formality process of job offer took a while b4 i joined (a gap of 1.5months)

how shoud i address this issue in the response to question no.1

please advice..in addition what other documents should i provide. did anyone of you get this kind of rfe b4?

submit offer letter, h1 approval notice, i94 copies, W2. if you are consultant DO NOT list your client name because they are not your employers.
please advice..in addition what other documents should i provide. did anyone of you get this kind of rfe b4?
